Apitegromab

Scholar Rock's myostatin-blocking antibody. Myostatin is the signal telling your body to limit muscle — block it, and the hope is you keep muscle you'd otherwise lose. Being tested alongside GLP-1 drugs so the weight you drop is fat, not muscle.

What people claim

Take it with a GLP-1 and your body keeps the muscle while burning the fat — the missing half of the weight-loss stack.

Human evidence

Genuinely real human data, unusual for anything discussed in muscle-preservation circles. Apitegromab has been through years of human trials in spinal muscular atrophy — including a positive phase 3 (SAPPHIRE) — which is where most of its human safety record comes from. For the use everyone here cares about, the phase 2 EMBRAZE trial paired it with tirzepatide in adults with obesity: similar total weight loss, with substantially less of that loss coming from lean mass. Phase 2 is promising, not proof — the bigger trials are what settle it.

Animal evidence

Myostatin inhibition has decades of striking animal biology behind it — famously double-muscled cattle and 'mighty mice' with natural myostatin mutations. That biology is why drug companies keep coming back to this target.

Risk flags

  • Long-term effects unknown
  • Unregulated / grey-market supply
  • Purity & quality unknowable
  • Banned in tested sport (WADA)

Regulatory status

US: Investigational. NOT FDA-approved for anything as of early 2026 — its SMA application hit a manufacturing-related delay, and the weight-loss use is still in trials.

UK: Investigational; no MHRA authorisation.

Who says what

Gym Bros Say

"This is the cheat code — GLP-1 melts the fat, myostatin blocker guards the muscle. The final boss of body recomp."

Reddit Says

GLP-1 subs are extremely excited about EMBRAZE, with a running debate about whether lean-mass preservation on a DEXA scan translates into actual strength and function.

Big Pharma Says

Scholar Rock is openly positioning apitegromab as the muscle-preservation partner for the GLP-1 era — the EMBRAZE trial with tirzepatide is exactly that play.

Science Actually Says

Grade B: real human trials across two indications, including a positive phase 3 in SMA and an encouraging phase 2 in obesity. The muscle-preservation-during-weight-loss claim specifically rests on phase 2 data — genuinely promising, not yet settled.

PeptideStackers Says

One of the three antibodies we think could genuinely change the fat-loss game. Also completely unobtainable legitimately right now — and a monoclonal antibody is not something a research-chem site can actually make, so treat any vial claiming to be this as fiction.

Honesty section

What we still don't know

  • ?Whether the preserved lean mass means preserved strength and function, not just a better DEXA readout.
  • ?Long-term safety of chronic myostatin blockade in otherwise healthy adults.
  • ?Whether the phase 2 obesity result holds up in larger, longer trials.
  • ?What happens to the preserved muscle after both drugs stop.

Real questions people ask

FAQ

Is apitegromab approved?
No. As of early 2026 it isn't approved for anything. Its lead application (for spinal muscular atrophy) was delayed by a manufacturing issue, and the obesity/muscle-preservation use is still in trials.
Can you buy apitegromab?
Not legitimately. And unlike small peptides, a monoclonal antibody requires living-cell bioreactor manufacturing and a cold chain — a grey-market vial labelled 'apitegromab' is almost certainly not a functioning antibody. This is the rare compound where the fakes aren't just risky, they're near-guaranteed.
Does apitegromab burn fat?
No — it's not a fat-loss drug. The idea is that it protects muscle while something else (a GLP-1 drug, a calorie deficit) drives the fat loss. That's why it's being trialled alongside tirzepatide, not instead of it.

Questions to ask a qualified professional

  • 01Is any approved, monitored option available to me for preserving muscle during weight loss (protein intake, resistance training, supervised care)?
  • 02How would a doctor even verify that an unapproved antibody product is real?
  • 03If this gets approved, what monitoring would come with it?
